Well, that's where you gotta start. First you think of the thing you can't get away with saying (or drawing), then you come up with a way of repackaging it so it still has all of its sizzle, but is in a form that no one can object to.
I'm not just talking about low humor or cheesecake here, but also serious painting and fiction, too. There has to be tension and conflict and excitement. Murder mysteries start off with a dead body. That's not nice! But it's the most popular fiction genre there is.
